P0AFA
Hybrid/EV battery system voltage low
What it means
The controller reports low or implausibly low traction-battery voltage. This may reflect genuinely low voltage or a sensing-circuit fault; on many Toyota/Lexus hybrids the monitor specifically checks for an open sensing circuit.
Symptoms
- Hybrid/EV warning light
- Car may not go into READY or may shut down
- Reduced power
Likely causes, most likely first
| No. | Cause |
|---|---|
| 01 | open or corroded battery-voltage sensing wiring or connections |
| 02 | fault in the battery voltage sensor, smart unit or battery ECU |
| 03 | genuinely low pack voltage or weak modules, where supported by the model-specific monitor and other codes |
| 04 | battery interconnection or charging fault, depending on the vehicle |
What to check, step by step
- 01Check the 12V battery, then save battery codes, subcodes and freeze-frame data
- 02Follow the OEM definition and compare reported block voltages for implausible or missing readings
- 03Have a high-voltage qualified technician test sensing wiring, connections and the battery controller under the OEM isolation procedure
- 04Confirm genuine discharge before attempting recovery charging or repeated engine starts; use only the specified recovery procedure
